Qualifying Process

Qualification for the Michigan Medical Marijuana Program is an easy process once you have contacted the Medical Marijuana Card Doctors. While we must follow Michigan law, we still make the medical marijuana application process a quick and easy one. Following are the steps in the qualification process.

First, you must have a medical condition that qualifies you for the use of medical marijuana under Michigan Laws and Statutes.


Qualifying Conditions:

Below is the list of current qualifying conditions in the State of Michigan. It is important to remember that additionals conditions will be added in the very near future. So please contact us at anytime so we can update you about your condition and how we might be able to assist you in getting your Medical MArijuana Card.

• Amyotrophic Lateral Sclerosis
• Agitation of Alzheimer's Disease
• Cachexia or Wasting Syndrome
• Seizures
• Muscle Spasms
• Glaucoma
• Hepatitis C
• Crohn's Disease
• Nail Patella
• Severe and Chronic Pain
• Epilepsy
• Severe Nausea


Step One - Medical Records:

The first step in the process is  documentation of your medical condition. Without current  medical records, our physicians will be unable to certify your condition under Michigan law. Although there are conditions where a patient will qualify for the Michigan Medical Marihuana Program without the necessity of full documentation,we require that all your records are current within the last three years that document your qualifying condition.

Step Two - Medical Record Review:

Our physicians will review your records to determine if you qualify for the Michigan Medical Marihuana Program. If additional information is required, we will contact you and let you know what is needed for our records. If you qualify for the program, we will call you to schedule an appointment with one of our physicians

Step Three - Certification Appointment:

Our physicians will talk with you about your qualification for certification of a medical condition that may qualify you for a Michigan Medical Marijuana Card. By law, we can only discuss your qualification for the program. After  approving your condition, qualified patients will receive a signed Physician Certification Form that will allow them to apply to the state for a Medical Marijuana Identification Card. In order to receive the Physician Certification Form, patients must present a valid Michigan ID and the physician visit fee (if you are on SSI/SSD or Medicaid the fee will be $125).

Step Four - Application for your Michigan Medical Marijuana Card:

In order to receive your Medical Marijuana Identification Card, you must send the following information to the Michigan Department of Community Health or we can do this for you:

» Signed and dated State Medical Marijuana Application Form
» Physician’s Certification
» Caregiver Attestation (if applicable)
» Photo ID
» $100 Application Fee

The information you provide will be verified within 15 days of receiving the application. If approved, your Medical Marijuana Identification Card will be issued and sent to the address you provide.
